Featured member, Ammonoosuc Community Health Services, has been a cornerstone of accessible healthcare in the North Country for half a century, providing comprehensive, patient-centered care to families across northern New Hampshire regardless of their ability to pay. Known throughout the region as ACHS, this nonprofit community health center serves as a true medical home for tens of thousands of Granite Staters.

Fifty Years of Caring for the North Country

Ammonoosuc Community Health Services was established in 1975, born from a community-driven effort to ensure that rural residents had reliable access to quality medical care. In the decades since, it has grown from a small local clinic into a regional network of care sites serving more than 26 towns across northern New Hampshire. Today, ACHS operates four care sites across northern New Hampshire, located in Littleton, Warren, Woodsville, and Whitefield.

A defining milestone came in 1994, when ACHS achieved status as a Federally Qualified Health Center, or FQHC. That designation enabled the organization to offer its full range of services to every member of the community, regardless of social or economic circumstances, cementing a commitment to care for all that continues to define its mission today.

Comprehensive Care Under One Roof

What sets ACHS apart is its integrated, whole-person approach to health. Rather than treating medical needs in isolation, the organization brings together a wide spectrum of services designed to care for the entire family at every stage of life.

Patients have access to patient-centered primary care for all ages, delivered by experienced providers who take the time to listen. Behavioral health and counseling services are offered alongside medical care, reflecting the understanding that mental health is just as important to overall well-being as physical health. ACHS also works in conjunction with area eye doctors to make vision exams and glasses affordable, and offers medication-assisted therapy, outreach, and treatment for individuals working toward recovery.

Rounding out its services is a team of patient navigators who act as personal healthcare assistants, helping patients meet their health goals and manage chronic conditions such as diabetes and hypertension. This coordinated model ensures that no one has to navigate the healthcare system alone.

Care for Everyone, Regardless of Circumstance

At the heart of ACHS is a simple but powerful principle: no one is turned away based on an inability to pay. As a nonprofit federally qualified health center, ACHS provides evidence-based, outcome-specific care that is patient-centered, prevention-focused, accessible, and affordable to all.

This commitment makes ACHS an essential safety net in a rural region where access to care can be a genuine challenge. For many families across the North Country, the organization is not just a clinic but a lifeline, offering dependable, high-quality care close to home.

A Mission Rooted in Community

ACHS describes its mission clearly: to provide a network of comprehensive primary health care and support services to individuals and families throughout the communities it serves. That community orientation runs in both directions. Just as ACHS serves the region, it also depends on the region’s support.

As a 501(c) nonprofit, ACHS relies on a combination of state and national funding along with the generosity of community members and individual donors. Contributions directly support the programs and services that benefit friends, neighbors, and families throughout the North Country, making local giving a tangible way to strengthen healthcare access across the region. Like many rural health centers nationwide, ACHS continues to navigate real financial pressures, which makes that community support more meaningful than ever.

Food as Medicine

ACHS believes good health starts well before a doctor’s visit, and its Wellness and Nutrition program puts that belief into practice. Built on the idea that better nutrition can make a real difference in overall health, the program helps patients understand how the foods they eat affect chronic conditions such as diabetes, high cholesterol, and high blood pressure, and teaches practical skills for shopping, preparing, and cooking healthier meals.

The program is refreshingly accessible. ACHS produces a library of free cooking videos, nutritious recipes, and easy-to-follow wellness tips that anyone can benefit from, whether they are managing a health condition or simply looking to eat better. From a berry-packed super smoothie to sautéed Swiss chard and lightened-up dips, the recipes are approachable, affordable, and genuinely useful. The organization also shares straightforward guidance, like a simple rule for choosing grain products with a healthier fiber-to-carbohydrate ratio.

ACHS extends this work into the community through outreach, bringing health and wellness demonstrations to local schools, businesses, and nonprofits, either in person or virtually. The program is made possible in part through partnerships with community organizations, reflecting the collaborative spirit that runs throughout the North Country.

Your Health, Close to Home

For anyone in the region looking for a primary care home, behavioral health support, or help managing a chronic condition, ACHS welcomes new patients and works to make becoming a patient as straightforward as possible. Its providers are dedicated to delivering quality healthcare and exceptional service, treating each patient as a neighbor rather than a number.

After fifty years of service, Ammonoosuc Community Health Services remains a vital part of what keeps the North Country healthy, a locally rooted organization built on the belief that quality healthcare should be within reach for everyone.
