Ammonoosuc Community Health Services (ACHS) is a Federally Qualified Health Center providing comprehensive primary care services for all ages in New Hampshire’s North Country. With locations in Littleton, Whitefield, Warren, Woodsville, and Franconia, ACHS delivers convenient, affordable, high-quality care to the communities we serve.
ACHS offers family medicine, pediatrics, behavioral health, substance abuse treatment, oral health services, and care management. Our experienced, compassionate providers treat the whole person, promoting wellness and chronic disease management. Sliding-fee discounts based on income are available to ensure access for all residents.
In addition to primary care, ACHS provides a range of community health programs including nutrition counseling, diabetes education, and tobacco cessation. We are committed to improving the health and well-being of the region through prevention, education, and collaboration with local partners.
ACHS accepts most insurances including Medicare and Medicaid and offers enrollment assistance for the Health Insurance Marketplace and NH Medicaid. New patients are welcome at all locations, with appointments available Monday through Friday.
